8/17 –	Presents with bridge #4-11 out. Abutments #’s 6,7,8 and 11 fractured at or slightly above the c.e.j. (Previous dentist tried to “tap out” bridge) Treatment at this time, re-cement with C&B Metabond.
8/18 -	Next day, endo on #’s 6,7,8 and posts cemented. Old bridge recemented with Panavia-F and models taken for temporary bridge.
9/14 -	Endo, post and rebuild on #11. Core rebuild on #’s 6, 7 and 8. Crown lengthening on #’s 6, 7, 8 and 11 with laser and prep for crowns. Cement temporary bridge #4-11 with Zone.

10/05 -

Impression for permanent crowns after margins finalized and records for articulation. (Face bow eccentric movements-centric) #12 MOD Composite. Recement temporary with Zone.

10/26 -

Crown try-in. Reshaped #’s 6-8 to move incisal edge lingually and then porcelain buccally and lingually. Checked phonetics, esthetics and occlusion.
Final Impression for partial upper with crowns inserted. Recemented temporary bridge.

11/09 -

Checked occlusion and esthetics on crowns and framework fit of partial upper. Check bite.

Color perfect. Delivered crowns and partial upper. Cemented with Panavia-F. Occlusal adjustment for eccentric movements. Esthetics and phonetics fine.
